Re-Enrollment Procedures
Please download and print the needed forms. Fill in the information for your child(ren) and send all documents and the re-enrollment fee to the school (please write "re-enrollment" on the outside of the envelope), or you may drop it off at either campus.
Remember that if any of these items are not received at the time of re-registration, your paperwork will be placed on hold. As in years past, there will be 4 forms for re-enrollment;
Change of Address Form
Please submit this form if you have changed your address recently so we may update our information.
Non Returning Student Form
This form will serve as written notice for non-returning CMCS student(s). Please complete and return the form to the school office.
